District Forms The forms linked here will help club Presidents, Secretaries, and Lt. Governors submit required information to the District office (and others as needed) in a timely manner. Association Bylaws If you have technical problems or questions, email the Webmaster How to use Monthly Report form in Word format: To save:  In Internet Explorer, RIGHT click on the form name and select "save target as".  In Netscape, right click on the form name and select the "Save link as".  Place the file on your hard drive in a place you can easily find.  The fields on the form are locked and you will open it as read only.  Fill it out and when complete select "Save as" from the File menu.  Give it a unique name (perhaps one that identifies your club).  Once the file is saved, select "Send to" from the file menu and select "Mail Recipient".  When your mail program comes up, fill in the address to alexia@gakiwanis.org.  Now your monthly submission is complete. NO PAPER FORM IS REQUIRED BY THE DISTRICT OFFICE.  Make sure you keep a backup copy of the form (file).
